Enzalutamid als Standard First-Line Therapie bei metastasierendem Prostatakrebs

METASTATIC PROSTATE CANCER Melburne – mechentel news – In dieser ersten der beiden im NEJM veröffentlichten Studien (ENZAMET und TITAN) zum Thema der medikamentösen Therapie des hormon-sensitiven metastasierten Prostatakrebses präsentiert das Team um Ian D. Davis aus dem Peter MacCallum Cancer Centre and the University of Melbourne (S.K.S., S.G.W.) in Melbourne, zuerst die Ergebnisse der „ENZAMET“ Studie. Die Forscher randomisierten in dieser open label Phase III-Studie 1.125 Männer: Gruppe 1 erhielt eine Androgendeprivation plus Enzalutamid, die Gruppe 2 eine Standardtherapie mittels Antiandrogen. Der primäre Endpunkt war das Gesamtüberleben, der sekundäre Endpunkte beinhaltete (PSA-)progressionsfreies sowie klinisches progressionsfreies Überleben. Zudem wurde das Auftreten von Nebenwirkungen erfasst. Das mediane Follow-Up betrug 34 Monate. Das Gesamt Überleben i n den Kaplan-Meier Kurven nach 3 Jahren betrug 80 % in Gruppe 1 und 72 % in Gruppe 2. Auch stellten die Autoren in der Juli-Ausgabe der Fachzeitschrift THE NEW ENGLAND JOURNAL OF MEDICINE fest, dass im PSA-progressionsfreien Überleben (HR 0,39, P < 0,001) und im klinisch progressionsfreien Überleben (HR 0,4
P < 0,001) ebenso ein Vorteil für Gruppe 1 zeigen liess. Therapieunterbrechungen aufgrund von Nebenwirkungen waren häufiger in Gruppe 1 (33 versus 14 Fälle). (cw/um)
